Who can write a letter of medical necessity?

A licensed clinician who has evaluated you and is acting within their scope of practice.

Plan administrators generally expect the letter to come from a treating provider, such as a physician, nurse practitioner, or other licensed professional who has a real care relationship with the patient.

The letter should identify the clinician by name, license, and NPI so the administrator can verify who issued it.

Pending clinician sign-off. General education only, not medical, tax, or legal advice. Your plan administrator decides eligibility.
