What is a letter of medical necessity (LMN)?
A letter from a licensed clinician explaining why a specific treatment, service, or item is medically necessary for a specific patient's condition.
An LMN is written by a clinician who has evaluated the patient. It typically names the condition, describes the recommended care or item, and explains why it is needed to treat, mitigate, or prevent that condition.
FSA and HSA administrators may ask for an LMN when an expense could be either medical or personal, because eligibility depends on the purpose of the expense. The administrator, not the clinician or MedsClaim, makes the final decision.
Pending clinician sign-off. General education only, not medical, tax, or legal advice. Your plan administrator decides eligibility.
